Hospital journey tracker project with Alzheimer’s Society
Last year, Phew teamed up with the Alzheimer’s Society’s Innovation Team to develop a new much-needed hospital journey tracker, to help reduce the amount of time that people living with dementia spend in hospital, unnecessarily, and offer transparency of patient’s care.
The tracker, which was co-designed with people affected by dementia and health and social care professionals, and developed within the Phew Patient Discharge Management System, sends email status updates to a named patient representative to inform them of what is happening now in a patient’s care, what is expected to happen next, an anticipated discharge date and when the patient was discharged.
It was first implemented at Luton and Dunstable University Hospital, but due to its success, the tracker is also being used at Bedford Hospital.
Alzheimer’s Society recently shared a story of Tiziana’s mother, who had been affected by dementia, and the miscommunications she endured relating to her mother’s hospital discharge, and how this led Tiziana to joining a focus group which helped Phew and Alzheimer’s Society develop the hospital journey tracker.
